Measurement of intradiscal pressure is instrumental in explaining, from a mechanical point, the occurrence of posterior ruptures in the lumbar discs, and provides a basis for the rational treatment of patients with low-back pain in so far as these exhibit increase of pain on increased mechanical loads. Intravitally-performed measurements of disc pressure over the last 20 years in more than 100 individuals have demonstrated how the load on the lumbar disc varies with the position of the subject's body and during the performance of various tasks, both in standing and in sitting. We referred to data from Nachemson and from Sato et al. regarding intradiscal pressure and to the methods of Shirazi-Adl for data for other material characteristics (see text for these references). The mid-position model bears a load of 294 N in the vertical direction, while the models of the flexed and extended positions bear loads of 15 N-m. An interesting observation was that changes in intradiscal pressure appeared to be minimal until a threshold distraction tension was reached.
Studies, from Japan, Sato ( Sato et al., 1999 ) , using a piezoresistive sensor in the transducer needle detecting the pressure laterally at a window and not at the needle tip, confirmed the pioneering findings of Nachemson and examining the changes in the more relevant L4/5 disc rather than the L3/4. Intradiscal pressure under axial compression Axial compression exhibited a strong decrease in the intradiscal pressure from the upper cervical to the upper lumbar region, from where the pressure remained almost constant towards the sacrum (Fig. 3).

In vivo measurements of intradiscal pressure;: Discometry, a method for the determination of pressure in the lower lumbar disc intradiscal pressure measurements showed that distraction tension routinely applied by the VAX-D equipment reduced the intradiscal pressure significantly, to negative levels in the range of-100 to -160 mm Hg. Even the famous Alf Nachemson studies on disc pressures showed increased Hg (on discs, which I equate with increased loading on the spine) with flexed postures, esp. slumped sitting or leaning forwards seated into flexed posture. It does not have anything to do with intradiscal pressures.
